Nutrition at 75: Why B12 Absorption Falls and What to Do
Vitamin B12 absorption from food falls with age, because stomach acid production declines in a large share of older adults. Vegetarians over 70 face a double risk — less B12 in the diet and less absorbed — so a blood test is worth asking for.
- An estimated 10–30% of older adults absorb B12 from food poorly.
- The cause is reduced stomach acid, often from atrophic gastritis.
- Vegetarian diets supply little B12, so the risk compounds.
- Metformin and long-term acid-reducing tablets lower B12 further.
- ICMR-NIN sets the adult RDA at 2.2 µg a day.

Why absorption falls
B12 in food is bound to protein. Stomach acid is needed to release it before it can be absorbed. In many older adults, a condition called atrophic gastritis reduces acid production, so B12 from food is absorbed poorly even when the diet contains enough.
The US Institute of Medicine estimated that 10–30% of older adults may have this kind of food-bound B12 malabsorption, which is why it recommends that older adults get much of their B12 from fortified foods or supplements, whose B12 is not protein-bound.
Why vegetarians are at double risk
B12 occurs naturally almost only in animal foods. A vegetarian diet relies on milk, curd and paneer, which contain modest amounts. B12 deficiency in Indian vegetarians is common at every age; after 70, reduced absorption adds to a low intake.
| Food | approximate B12 |
|---|---|
| 1 glass milk (250 ml) | About 1 µg |
| 1 katori curd | About 0.5–1 µg |
| 50 g paneer | About 0.4 µg |
| 1 egg | About 0.5 µg |
| Dal, vegetables, grains | Essentially none |
Values are approximate. Against an RDA of 2.2 µg, a vegetarian diet can reach the target on paper while delivering much less once absorption is accounted for.
Medicines that lower B12
Long-term metformin for diabetes and long-term acid-reducing tablets (PPIs) both reduce B12 levels. nutrition at 72 covers these interactions. If you take either, ask whether your B12 should be checked.
Signs, and what to ask for
- Tingling or numbness in hands and feet.
- Unusual tiredness or breathlessness.
- Memory problems or low mood.
- A sore, smooth tongue.
These symptoms have many causes, and B12 deficiency can be present without them. A simple blood test settles it. If B12 is low, a doctor will advise oral or injected B12 depending on the cause and severity. Vitamin B12 deficiency has more detail.
Why it matters for memory
Untreated B12 deficiency can cause nerve damage and cognitive problems, and some of this becomes permanent if left too long. That is a strong reason to test rather than assume. nutrition at 77 covers diet and memory more broadly.
Where KABO fits in
Low B12 needs a blood test and, if confirmed, a doctor’s treatment plan. No shake replaces either.
Fortified foods are one of the routes the Institute of Medicine points older adults towards, because their B12 is not protein-bound. KABO is one such product: A KABO serving provides 23–25 g of complete plant protein, 26 vitamins and minerals, 4 g of fibre and 8 billion probiotics in about 198 kcal, from 60+ ingredients; it is vegan and lactose-free. Check your B12 level with a doctor rather than relying on it. If you follow a kidney-related diet restriction, check with your nephrologist before adding any protein product, and if you take several medicines, show the label to your doctor or pharmacist first — fortified products can contain vitamin K, which matters on warfarin.

Frequently asked questions
Why do older people become deficient in vitamin B12?
Many older adults produce less stomach acid, often because of atrophic gastritis, and acid is needed to release B12 from food. The US Institute of Medicine estimated that 10 to 30 percent of older adults absorb food-bound B12 poorly. Medicines such as metformin and acid-reducing tablets add to the risk.
How much vitamin B12 does a 75-year-old need?
ICMR-NIN sets the adult RDA at 2.2 micrograms a day. Because absorption from food falls with age, older adults, especially vegetarians, may need fortified foods or a supplement, ideally after a blood test.
Can vegetarians get enough B12 after 70?
It is difficult from food alone, because dairy and eggs contain modest amounts and absorption declines with age. A blood test and, if needed, a supplement advised by a doctor is the reliable approach.
What are the symptoms of B12 deficiency in elderly people?
Tingling or numbness in the hands and feet, tiredness, breathlessness, memory problems, low mood and a sore tongue. Deficiency can also be present without symptoms, so testing is more reliable.
Does metformin affect vitamin B12?
Yes, long-term metformin use is associated with lower B12 levels. Periodic testing is often recommended for people who have taken it for years. Do not stop metformin without medical advice.
